I have the same problem, the seal catches on the overlap on the inside B post. Only thing that works it talc, athough it only lasts a couple of days. Slack main cable is a possibility as stated, the terminals fatigue with age and open up leading to poor connections.
If your vans cuts out with fuse out its wired wrong as the ignition circuit is not fused.

Sorry mate, the original hazard switch wont work with a modern hazard relay, a later bay switch would work if you modified the wiring accordingly. On the original system the hazard switch activated the hazard relay that outputted to all the indicators, on later bay systems with the modern style haza...

It's a very common problem, sounds like you've got the fuse box wired wrongly. The ignition circuit should be unfused i.e. if the ignition fuse blows you'll lose brake lights, indicators and wipers(depending on age) but not the ignition. The permanent live fuse is where the feed for the lights comes...
